Background

Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the use of the term “pragmatic” is not uniform and its definition and assessment requires further clarification. Pragmatic trials should be designed to guide clinical practice and policy decisions, rather than confirm the validity of a clinical or physiological hypothesis. A pragmatic trial should also try to be as similar to the real-world clinical environment as possible, such as the participation of participants, setting up and design of the intervention, its delivery and execution of the intervention, and the determination and analysis of outcomes and primary analyses. The trials that are truly pragmatic must be careful not to blind patients or clinicians, as this may result in bias in estimates of the effect of treatment. Practical trials should also aim to enroll patients from a wide range of health care settings to ensure that their findings are generalizable to the real world.

Furthermore, trials that are pragmatic must focus on outcomes that matter to patients, like quality of life and functional recovery. This is especially important in trials that involve invasive procedures or those with potentially serious adverse events. Methods

In a practical study, the goal is to inform clinical or policy decisions by showing how an intervention could be integrated into routine care in real-world contexts. Explanatory trials test hypotheses concerning the cause-effect relation within idealized settings. Consequently, pragmatic trials may have less internal validity than explanatory trials, and could be more susceptible to bias in their design, conduct and analysis. Despite their limitations, pragmatic research can be a valuable source of information to make decisions in the healthcare context.

The PRECIS-2 tool assesses the degree of pragmatism in an RCT by assessing it on 9 domains, ranging from 1 (very explanatory) to 5 (very pragmatic). In this study, the recruitment, organization, flexibility in delivery, flexible adherence and follow-up domains were awarded high scores, however the primary outcome and the procedure for missing data were below the practical limit. This suggests that a trial can be designed with well-thought-out practical features, but without harming the quality of the trial.

The framework consisted of nine domains that were assessed on a scale of 1-5, with 1 being more explanatory while 5 was more pragmatic. The domains covered recruitment and setting up, the delivery of intervention, flexible adherence and primary analysis.

The original PRECIS tool3 had similar domains and a scale of 1 to 5. Koppenaal and colleagues10 developed an adaptation to this assessment called the Pragmascope that was simpler to use in systematic reviews. They found that pragmatic systematic reviews had higher average scores in the majority of domains, but lower scores in the primary analysis domain.

This distinction in the analysis domain that is primary could be explained by the fact that most pragmatic trials analyze their data in an intention to treat manner while some explanation trials do not. The overall score was lower for pragmatic systematic reviews when the domains of the organization, flexibility of delivery and follow-up were combined.

It is important to remember that the term “pragmatic trial” does not necessarily mean a low quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but this is not specific nor sensitive) that use the term “pragmatic” in their title or abstract. The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism but it isn’t clear if this is manifested in the contents of the articles.

Conclusions

As the value of evidence from the real world becomes more widespread and pragmatic trials have gained momentum in research. They are randomized trials that evaluate real-world care alternatives to new treatments that are being developed. They include patient populations that are more similar to those who receive treatment in regular medical care. This method can help overcome limitations of observational studies, such as the biases that arise from relying on volunteers, and the limited availability and the variability of coding in national registry systems.

Other advantages of pragmatic trials include the possibility of using existing data sources, and a greater chance of detecting meaningful changes than traditional trials. However, these trials could still have limitations that undermine their validity and generalizability. For example the rates of participation in some trials could be lower than expected due to the healthy-volunteer effect and financial incentives or competition for participants from other research studies (e.g. industry trials). Practical trials are often restricted by the necessity to recruit participants on time. Additionally certain pragmatic trials don’t have controls to ensure that the observed differences aren’t due to biases in the conduct of trials.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-described themselves as pragmatic and that were published up to 2022. The PRECIS-2 tool was employed to assess the degree of pragmatism. It includes areas such as eligibility criteria, recruitment flexibility, adherence to intervention, and follow-up. They discovered that 14 of these trials scored as highly or pragmatic practical (i.e. scores of 5 or more) in any one or more of these domains and that the majority of them were single-center.

Studies that have high pragmatism scores tend to have more criteria for eligibility than traditional RCTs. They also contain populations from many different hospitals.
